The Butterfly Lifestyle aims to bridge the gap and make a healthy lifestyle accessible to everyone.
Eligibility
- NO COST: Gross income must be at or below 200% of the federal poverty levels for families
- NOMINAL COST: Gross income above 200% of the federal poverty levels for families (Not Applicable To All Programs)
Income Verification
All applicants will be required to provide a Government Issued ID along with proof of income such as:
- W-2 withholding statements or Unemployment check stubs for the past 90 days
- Paycheck stubs for the past 90 days for all persons employed in the home
- Proof of all other income received in the past 90 days
- Proof of government assistance
- Proof of Bankruptcy settlement (if applicable)

- The nonprofit will evaluate each request on a case by case basis to approve eligibility
- All applicants MUST meet with a representative from the nonprofit to provide income verification documentation
